1.  Sign up online
 
We strongly encourage you to register online.  However, all parade participants must sign a waiver!  If a participant is a minor, a parent or responsible adult must sign the waiver.

There are two ways to do this:  you can either
• download the waiver form as a PDF document, print it, fill in your name, sign and date it, and bring it to a rehearsal, or
• come to rehearsal before 4:00 pm, find Lisa Ruiz or Ted Syrett, get a waiver form from one of them, fill in your name, sign and date it, and hand it back.

2.  Sign up onsite
 
SF Carnaval 2011 rehearsals take place Saturday afternoons from 4:00 to 6:00 pm beginning April 2 at Senzala Restaurant, 251 E. Java Drive, Sunnyvale, between Borregas Avenue and Geneva Drive.  [Directions]
Just show up before 4:00 pm and ask for Lisa Ruiz or Ted Syrett.  One of those people will have a supply of preprinted registration and waiver forms.  Take one of each, fill in the registration information, sign the waiver and turn in both forms.
 

3.  Download, print and fill in forms
 
Wait:  why are you doing this when you could be using our handy online registration?  Perhaps you live outside the San Francisco Bay Area and would like to mail in your registration and waiver forms together with payment.  To do this, click on the links below to download the forms, then fill them in, sign the waiver, and mail them to the address shown.
